When your hand wakes you up at night

You wake in the small hours with a hand that feels strange β€” numb, tingling, buzzing, as if it has "fallen asleep." You shake it out, hang it over the side of the bed, maybe get up and flex your fingers, and slowly the feeling comes back. In the daytime things seem more or less normal, but the numbness keeps returning at night, and lately it's turned up when you're holding a phone, gripping the steering wheel, or reading. The thumb, index, and middle fingers seem to be the ones that go tingly.

If that pattern sounds familiar, it may be carpal tunnel syndrome (CTS) β€” one of the most common reasons people develop numbness and tingling in the hand. It can be uncomfortable and, when it disturbs sleep, genuinely wearing. The good news is that it's a well-understood problem with several recognised ways to manage it, from simple night splints to hand therapy and, in some cases, a small operation. This guide explains what carpal tunnel syndrome actually is, the warning signs that mean you should get it checked sooner rather than later, how conservative-care professionals approach it, what the research does and doesn't show, and when to seek an assessment.

What carpal tunnel syndrome actually is

The carpal tunnel is a narrow passageway on the palm side of your wrist. Its floor and sides are formed by the small wrist (carpal) bones, and a tough band of tissue called the transverse carpal ligament forms the roof. Running through this tunnel, alongside the tendons that bend your fingers, is the median nerve β€” the nerve that carries sensation from the thumb, index finger, middle finger, and half of the ring finger, and that helps power some of the muscles at the base of the thumb.

Carpal tunnel syndrome happens when that median nerve becomes compressed, or squeezed, inside the tunnel [1]. When the space gets crowded β€” from swelling, fluid, repetitive strain, or simply individual anatomy β€” the pressure on the nerve interferes with the signals it carries. That's what produces the classic symptoms: numbness, tingling, and pain in the thumb, index, middle, and part of the ring finger, often worse at night and sometimes easing when you shake the hand [1]. CTS is described as the most common of the "pinched nerve" conditions in the limbs [1], which is one reason it's so widely recognised.

Understanding the mechanism helps explain the symptoms. The reason it flares at night, for instance, is thought to relate partly to wrist position and fluid shifts during sleep β€” which is also why a splint that keeps the wrist straight is such a common first suggestion. And because it's a nerve problem rather than a joint or muscle strain, the sensations are typically numbness, tingling, and buzzing rather than a simple ache.

Warning signs that mean "get this checked"

Most carpal tunnel symptoms start out coming and going β€” tingling at night, numbness during certain activities, relief when you change position. Some signs, though, suggest the nerve is under more sustained pressure and deserve a prompt professional assessment rather than watchful waiting:

  • Numbness that is constant rather than coming and going. When the loss of feeling stops fluctuating and becomes steady, it can signal that the nerve compression is more advanced [1].
  • Weakness or clumsiness in the hand β€” dropping objects, struggling to grip, fumbling with buttons, keys, or doorknobs. Descriptions of untreated CTS note this loss of fine coordination and strength [1].
  • Wasting at the base of the thumb. The fleshy mound on the palm side below the thumb is called the thenar eminence. If it starts to look flatter or shrunken (thenar atrophy), that points to muscle involvement from longer-standing nerve pressure [1].

These are worth acting on because, once thumb-muscle wasting sets in, the condition is harder to reverse and may respond less well even to surgery [1]. In other words, the earlier median-nerve trouble is properly assessed, the more options you and a clinician tend to have. This is a prompt to get evaluated β€” not a way to grade your own nerve or diagnose yourself at home.

How the diagnosis is confirmed and why severity matters

A clinician diagnoses carpal tunnel syndrome from your history and an examination of the hand and wrist, but a nerve test often plays a central role. Electrodiagnostic testing β€” nerve conduction studies, sometimes with electromyography β€” measures how well and how quickly the median nerve is carrying signals across the wrist. It's considered the gold-standard test for confirming CTS [1], and a guideline-level review notes it also helps confirm the diagnosis in unusual presentations, rule out other causes, and gauge how severe the nerve involvement is [2].

Severity matters because it helps steer the plan. Carpal tunnel syndrome is often described in broad bands β€” mild, moderate, and severe β€” based on symptoms and nerve-test findings [1], and where you fall on that spectrum influences whether conservative measures are a reasonable first step or whether a surgical opinion should come sooner [2]. This is exactly the kind of judgement that belongs with a qualified professional who can put the test results together with your symptoms.

How conservative-care disciplines approach it

For many people with mild-to-moderate symptoms, non-surgical (conservative) care is a common starting point [2]. Two of the regulated disciplines most involved in that care are physiotherapy and occupational therapy. Neither "cures" a nerve, and the research below is presented honestly, including where it's thin. Importantly, this is not a ranking β€” the evidence doesn't crown one approach as best, and the right fit depends on your symptoms, their severity, and how you respond.

Physiotherapy

Physiotherapists typically begin by assessing the wrist, hand, and the nerve itself, and by identifying activities or postures that may be loading the median nerve. From there, conservative care often centres on wrist splinting β€” especially a splint worn at night that holds the wrist in a neutral (straight) position to take pressure off the nerve while you sleep.

Splinting is one of the better-supported first-line measures. A 2024 rapid evidence review for family physicians reports that night-only splinting appears to be as effective as wearing a splint continuously, and that a neutral splint may work better than one that holds the wrist bent back; it also notes that improvements typically show up within a couple of weeks [2]. The honest framing is that a splint is a way to ease symptoms and calm the irritation β€” a reasonable, low-risk thing to try β€” rather than a guaranteed fix for the underlying compression.

Physiotherapists may also use nerve-gliding (or nerve- and tendon-gliding) exercises, gentle movements designed to help the median nerve move more freely through the tunnel. Here the evidence is genuinely mixed and limited. A systematic review of randomised trials concluded that tendon- and nerve-gliding exercises may help when added to conventional care such as splinting, but have not been shown to work on their own, and called for better-quality studies [5]. The same guideline review is more cautious still, noting that adding nerve-gliding exercises to splinting did not improve outcomes in the evidence it examined [2]. So gliding exercises are a plausible add-on that some clinicians use, not a proven stand-alone treatment.

Occupational therapy

Occupational therapists focus on the tasks of daily life and work β€” how you use your hands, and how those demands might be feeding the problem. Their approach commonly includes activity and ergonomic changes (adjusting how you type, grip tools, or hold devices), advice on pacing and rest, and splinting, particularly custom or well-fitted night splints in a neutral position. Occupational therapists frequently manage the fitting, wearing schedule, and progress of these splints, drawing on the same evidence that supports night-time, neutral-position splinting as a sensible first-line measure [2].

Because occupational therapy is oriented around real-world function, much of its value is practical: reducing the specific strains that provoke your symptoms and helping you keep working and living with less aggravation, while more definitive decisions are made. As with physiotherapy, this is supportive, symptom-focused care β€” helpful for many people, but not a promise of resolving the nerve compression itself.

The corticosteroid injection question

You may also hear about a corticosteroid ("cortisone") injection into the carpal tunnel, aimed at reducing swelling and easing symptoms. The evidence here is worth laying out carefully, because different high-quality sources emphasise different things.

On the encouraging side, the 2024 family-medicine review reports that corticosteroid injections can give meaningful improvement in symptoms and function at around three months, with benefit lasting up to about six months, and may reduce the likelihood of needing surgery within the first year [2]. On the more cautious side, a 2024 systematic review and network meta-analysis found that while steroid injections produced a slightly greater improvement in symptom scores than splints or placebo injections in the short term, that difference was smaller than the threshold considered clinically important, and there was no clear difference by six months [4]. Those authors described the effect as a "minimal transient improvement" and suggested much of the benefit people feel may come from non-specific effects of being treated rather than the drug itself [4].

Putting those together honestly: a corticosteroid injection may offer some short-term relief, the size of that benefit is debated and may be modest, and it doesn't appear to durably fix the underlying problem. Whether an injection makes sense for you β€” given your severity, symptoms, and preferences β€” is a decision for a qualified clinician, not a default assumption.

When surgery is considered

For carpal tunnel syndrome that is more severe, or that keeps troubling you despite conservative care, a surgical carpal tunnel release may enter the conversation. The operation cuts the ligament forming the roof of the tunnel to make more room for the median nerve. The guideline review describes surgery as something considered when conservative measures have failed, or when there are moderate-to-severe symptoms together with signs of nerve injury on electrodiagnostic testing [2].

The evidence comparing surgery with splinting is nuanced, and worth understanding before any decision. A 2024 Cochrane systematic review found that, compared with splinting, surgery probably does not provide a clinically important benefit in symptoms or hand function β€” the average differences it measured were real but smaller than the amount considered meaningful to patients [3]. At the same time, surgery probably produced a higher rate of clinical improvement at 6 to 12 months of follow-up [3]. The Cochrane authors were also uncertain about how the two approaches compare on side effects, and concluded that the decision should weigh the modest benefits against the potential risks of an operation [3]. In plain terms: surgery helps more people reach "improved" over the following year, but on average the size of the benefit over splinting may be smaller than many expect β€” which is exactly why it's an individual decision made with a surgeon.

Self-management alongside professional care

While you're getting assessed and building a plan, several everyday measures are commonly suggested for carpal tunnel symptoms:

  • Try a night splint that keeps the wrist straight. Because symptoms often flare during sleep, a neutral-position splint worn at night is a widely used, low-risk first step, and night-only wear appears about as helpful as all-day wear [2].
  • Adjust aggravating activities. Notice which tasks set off the tingling β€” sustained gripping, bent-wrist typing, vibrating tools, long stretches on a phone β€” and modify how (and how long) you do them.
  • Take breaks and change position. Brief, regular pauses to straighten and move the wrist can ease the pressure that builds up during repetitive work.
  • Give measures time and track your response. Splinting benefits often appear within a couple of weeks [2]; keeping a simple note of your night-time symptoms helps you and a clinician judge whether an approach is working.

These are comfort-and-support steps, not a substitute for a proper assessment β€” especially if your symptoms are worsening.

When to seek an assessment

Carpal tunnel syndrome can share symptoms with other conditions β€” nerve irritation higher up the arm or in the neck, thyroid problems, diabetes-related nerve changes, and others β€” and these are managed differently. That's the main reason not to rely on self-diagnosis. It's worth having hand numbness or tingling assessed by a regulated health professional, and worth doing so promptly if:

  • The numbness has become constant rather than coming and going [1].
  • You notice weakness, clumsiness, or dropping things, or wasting of the muscle at the base of the thumb [1].
  • Symptoms are disturbing your sleep or interfering with work and daily activities despite trying simple measures.
  • The tingling or numbness appeared or worsened after a wrist injury, or comes with pain, swelling, or changes spreading up the arm.
  • You have another condition β€” such as diabetes or thyroid disease β€” or symptoms that don't fit the usual carpal tunnel pattern, so other causes can be ruled out [2].

This list is a prompt to get checked, not a tool to diagnose or grade yourself. A clinician can examine the hand, arrange nerve testing if needed, confirm what's actually going on, and help you weigh the options.

The bottom line, and a word on next steps

Carpal tunnel syndrome is common, usually well understood, and β€” especially when caught before the thumb muscles weaken β€” often manageable. Night splinting and activity changes are sensible, low-risk first steps that the evidence supports for easing symptoms; hand therapy adds practical, function-focused care; corticosteroid injections may help in the short term to a debated degree; and surgery is an option for more severe or persistent cases, with benefits and trade-offs worth discussing carefully. Across all of these, the research points to honest, realistic help rather than guaranteed cures, and it doesn't declare a single approach best for everyone.
