Plantar fasciitis: what it is, why it happens, and the care options

If the worst heel pain of your day arrives with your very first steps out of bed, you already know how disruptive it can be. Many people describe it the same way: a sharp, bruise-like ache under the heel that bites when they stand up in the morning or after sitting for a while, then eases a little once they get moving — only to creep back by the end of a long day on their feet.

That pattern is the classic picture of plantar fasciitis, one of the most common causes of heel pain. It is uncomfortable and can wear on you, but here is a reassuring place to start: for most people it is not a sign of serious damage, and it tends to settle over time with sensible care. This guide explains what plantar fasciitis is, why it happens, how different kinds of practitioners approach it, what you can try yourself, and when it is worth getting a professional assessment.

What plantar fasciitis actually is

The plantar fascia is a thick, fan-shaped band of tissue that runs along the sole of your foot, from the heel bone to the base of the toes. It works a bit like a spring and a shock absorber, supporting the arch as you stand, walk, and push off.

The name "fasciitis" ends in "-itis," which usually means inflammation. But that label may be misleading. When researchers examined tissue from people with long-standing heel pain, they found changes that looked more like wear and breakdown of the fibres — degeneration — rather than the classic signs of inflammation [1]. Because of this, many clinicians now describe the problem as a fasciosis or, more neutrally, a plantar fasciopathy — a change in the quality of the tissue rather than a simple "-itis" [1][2]. This is the current understanding, and it matters, because it helps explain why anti-inflammatory approaches alone often are not the whole answer.

Why does it happen? In most cases there is no single dramatic cause. The tissue is thought to become overloaded — asked to do more than it is currently conditioned for. Reviews of the condition consistently link it to a handful of contributing factors, including limited ankle flexibility (a tight calf), higher body weight, and jobs or activities that involve a lot of standing, walking, or running [2]. It is common in runners and in people who spend long shifts on hard floors. Often it is simply a mismatch between load and capacity that builds up gradually.

Plantar fasciitis is also very common — estimates suggest roughly one in ten people experience it at some point in life — so if you have it, you are in large company [2].

How different practitioners approach it

There is no single "right" path for plantar fasciitis, and different regulated professionals bring different tools and training. Below is an honest look at how each discipline tends to approach it, and what the evidence does and does not show. A useful thing to know up front: across the board, the effects of individual treatments are generally modest, and most approaches are described as helping you manage and improve — not as a guaranteed fix.

Physiotherapy

Physiotherapists usually start by looking at how your foot, ankle, and calf move and load, then build a program around stretching, strengthening, and gradually rebuilding the tissue's tolerance for activity.

Stretching is one of the better-supported starting points. A clinical practice guideline from the American Physical Therapy Association's orthopaedic and sports sections recommends plantar fascia–specific stretching together with calf (gastrocnemius and soleus) stretching to help reduce pain and improve function in the short and longer term [3]. Family-medicine guidance echoes this, noting that plantar fascia stretches are effective for reducing heel pain, and that stretching aimed specifically at the fascia may work better than calf stretching alone [4].

Physiotherapists may also use hands-on ("manual") techniques on the calf and foot, taping, and — for morning pain in particular — a night splint worn for a period of weeks to keep the tissue gently stretched overnight [3]. Some clinics also offer extracorporeal shockwave therapy (ESWT), a device that delivers pulses of energy to the heel. The evidence for shockwave is genuinely mixed: some reviews find it helps more than a placebo, while an influential earlier systematic review found any benefit for morning pain was very small and was no longer statistically clear when only the highest-quality trials were counted [7]. It is reasonable to view shockwave as an option that may help some people, rather than a sure thing.

Podiatry

Podiatrists focus on the foot and its mechanics. Their approach often includes assessing your gait and footwear, calf and fascia stretching, taping, and — commonly — foot orthoses (shoe inserts), either off-the-shelf or custom-made.

The evidence on orthoses is worth understanding honestly. A systematic review and meta-analysis of 20 trials concluded that foot orthoses are not clearly better than sham inserts or other conservative care for improving pain and function in plantar heel pain [5]. Notably, custom-made orthoses did not outperform prefabricated ones in that analysis [5]. The physiotherapy guideline reaches a similar practical conclusion: orthoses are not recommended as a stand-alone fix for short-term relief, but they may be worth using alongside other treatments such as stretching [3]. In plain terms, an insert on its own is unlikely to be the whole answer, but it can be a reasonable part of a broader plan, and a cheaper prefabricated pair may do as much as a custom one.

Podiatrists (and physicians) may also discuss a corticosteroid injection for stubborn, painful cases. Here the evidence calls for a careful, honest framing. A systematic review and meta-analysis found that corticosteroid injection can offer short-term pain relief, but was similar to a placebo injection for pain in the short and medium term — suggesting much of the benefit may come from non-specific effects rather than the drug itself [8]. Injections also carry real, if uncommon, risks, including rupture of the fascia and thinning of the protective fat pad under the heel [1][2]. This is a decision to weigh individually with a regulated professional, not a routine step.

Chiropractic care

Chiropractors who treat foot pain typically combine hands-on care with exercise and advice. Practitioners in this field approach plantar fasciitis with techniques such as joint mobilization or manipulation of the foot and ankle (often aiming to improve ankle flexibility), soft-tissue work on the calf, and stretching and loading programs.

The best-supported thread here overlaps with physiotherapy: manual therapy directed at the calf and foot, combined with stretching, appears to help more than stretching alone in the short term. In one randomized trial, adding trigger-point manual therapy targeting the calf muscles to a self-stretching program produced greater improvements in pain and physical function than stretching by itself [6]. Studies of manual therapy for this condition are generally small and short-term, so the honest summary is that hands-on care combined with an active exercise program is a reasonable option some people find helpful — most strongly supported when paired with stretching and loading, rather than as a treatment on its own.

Massage therapy

Massage therapists use hands-on soft-tissue techniques with the goal of easing tension in the calf and foot, improving comfort, and supporting your ability to keep moving. Practitioners in this field often work on the calf muscles and the sole of the foot, sometimes alongside stretching advice.

The research that comes closest to testing this is the manual-therapy and trigger-point work noted above, where soft-tissue treatment of the calf added to a stretching program improved short-term pain and function [6]. Direct, high-quality evidence for massage as a stand-alone treatment for plantar fasciitis is limited, so it is best understood as a supportive, comfort-focused part of a broader plan — particularly when combined with the stretching and loading that has stronger support — rather than a stand-alone cure.

What you can try yourself, and what to expect

Alongside any professional care, a lot of the day-to-day management is in your hands. The steps below reflect the general direction of the guidelines and reviews above, and are gentle and low-risk for most people:

  • Stretch the fascia and the calf. Fascia-specific and calf stretching are among the better-supported self-care steps [3][4]. A physiotherapist can show you the specific plantar fascia stretch, which is often done seated by pulling the toes back toward the shin.
  • Ease the load, don't stop moving. Cutting back temporarily on the activities that flare your heel — while staying generally active — gives the tissue a chance to recover. Building activity back up gradually tends to work better than long rest followed by a sudden return.
  • Supportive, cushioned footwear. Shoes with good arch support and cushioning, and avoiding long stretches barefoot on hard floors, are commonly advised as simple, low-cost measures.
  • Consider a simple insert. An off-the-shelf insole may help as part of your routine; the evidence suggests a prefabricated pair can do as much as a custom one for many people [5].
  • A night splint for stubborn morning pain. If the first-step-of-the-morning pain is the worst part, a night splint worn for a few weeks is a guideline-supported option to discuss with a professional [3].

On timeline, patience is genuinely part of the treatment. Plantar fasciitis tends to be self-limiting: about 80% of people improve within 12 months with non-surgical care [2][4]. That is encouraging, but 12 months is also a long time to be uncomfortable, and for some people symptoms take longer or come and go. Improvement is usually gradual rather than sudden, and starting sensible care earlier tends to help. If you are doing the basics and not seeing any change over a reasonable stretch of weeks, that is a signal to get assessed rather than simply pushing on.

When to seek a professional assessment

Most heel pain is not an emergency, and much of it settles with the kind of self-care above. But it is worth getting checked by a regulated health professional if any of the following apply:

  • Your heel pain is severe, or it is not improving at all after several weeks of sensible self-care.
  • The pain followed a specific injury, or you felt a sudden "pop" or tearing in the sole or heel.
  • You have numbness, tingling, pins-and-needles, or burning in the foot — symptoms that can point to a nerve issue rather than the fascia.
  • There is swelling, redness, warmth, or a fever, or you feel generally unwell.
  • You have diabetes, poor circulation, or a condition that affects healing or sensation in your feet.
  • The pain is in both heels at once, especially if you also have joint pains elsewhere.

These are prompts to get assessed, not a way to diagnose yourself. Heel pain has several possible causes beyond plantar fasciitis — including nerve, bone, and other soft-tissue problems — and a proper history and examination is how those get sorted out.

A note on getting the right help

Plantar fasciitis is common, usually not serious, and tends to improve — but "usually" is not "always," and your feet are specific to you.
