If you have ever watched someone get a chiropractic adjustment, you probably remember the sound: a quick series of pops or cracks. Maybe you have wondered what is actually happening in there, whether it is supposed to sound like that, and what it would feel like if it were your back on the table. This guide walks through what a chiropractic adjustment is, what that popping noise really is, and what people commonly experience during and after. The goal is to help you feel informed, not to tell you what to do with your body. That is a conversation for you and a qualified practitioner.

What an adjustment actually is

A chiropractic adjustment is also called spinal manipulation. In plain terms, a chiropractor uses their hands (or sometimes a small handheld instrument) to apply a controlled, quick push to a specific joint in your spine or another part of your body.

The most common form has a technical name: a high-velocity, low-amplitude thrust. "High-velocity" means it is fast. "Low-amplitude" means the joint only moves a small distance. Put together, it is a quick, shallow, targeted push, usually taking the joint a little past its normal resting range but within the range it can safely move. The whole thing lasts a fraction of a second (Evans, 2002).

Before any of that happens, a good practitioner spends time understanding your history. They will usually ask about your symptoms, look at how you move, and feel along the area before deciding whether an adjustment is appropriate for you at all. Not everyone is a candidate, and a careful practitioner will say so.

So what is that popping sound?

This is the part almost everyone is curious about. For a long time, the cracking noise was a bit of a mystery, and there were competing theories about it.

One of the clearest looks at the question came from a Canadian research team led by Greg Kawchuk. In a 2015 study published in the journal PLOS ONE, they used real-time MRI to actually film a finger joint as it was gently pulled apart until it cracked. What they saw was this: as the joint surfaces separated, a gas-filled cavity rapidly formed inside the joint fluid, and the pop happened right as that cavity appeared. In other words, the sound seems to come from a cavity forming, not from a bubble bursting (Kawchuk et al., 2015).

The researchers connected this to a process called tribonucleation, where two surfaces resist coming apart until they suddenly separate, leaving behind a gas cavity in the fluid between them. It is worth being honest here: this study looked at knuckle joints, not spines, and science on the exact mechanics is still developing. But it gives us a solid, evidence-based picture of where that familiar noise likely comes from.

A couple of practical takeaways follow from this. First, the pop is a normal byproduct of the joint separating, and it involves gas in the joint fluid, not bones grinding or anything breaking. Second, the sound itself is not the point of the treatment. An adjustment can happen without a pop, and a pop does not mean the adjustment "worked" better. Some techniques do not produce a sound at all.

Manipulation versus mobilization

You may hear two different words used, and they are not the same thing.

Manipulation is the quick thrust described above: fast, small, and taking the joint slightly past its usual range. It often produces that popping sound.

Mobilization is gentler and slower. Instead of a single fast thrust, the practitioner uses slow, rhythmic, back-and-forth movements to guide a joint through its natural range of motion. It is lower force, you can usually stop it at any moment, and it typically does not produce a pop (NCCIH).

Practitioners choose between them based on the person in front of them, that person's comfort, and their clinical judgment. Neither one is automatically "better." If the idea of a quick thrust makes you nervous, this is exactly the kind of thing to raise before you are on the table. Many practitioners can adjust their approach, and some people simply prefer the slower option.

What to expect during and after

During an adjustment, you are usually lying on a padded table positioned so the practitioner can reach the joint they are working on. The push itself is quick. Many people find it painless, and some describe a sense of relief or looser movement afterward. Others feel a brief moment of pressure. Everyone is a little different.

Afterward, it is common to feel some mild soreness. According to the U.S. National Center for Complementary and Integrative Health, transient mild-to-moderate side effects, most often increased soreness, stiffness, or a headache, can follow spinal manipulation or mobilization, and most of these fade within about 24 hours (NCCIH). Think of it as similar to the achiness you might feel after trying a new kind of exercise. It is a common, usually short-lived response, not a sign that something went wrong.

That said, "common and usually mild" is not the same as "risk-free," and honest information matters more than reassurance. Serious problems are considered rare, but they are why choosing a qualified, registered practitioner and sharing your full health history matters. A few sensible habits: mention any health conditions, medications, or recent injuries; speak up if something hurts during or after; and check in with your practitioner or another health professional if soreness lasts longer than a day or two or feels different from ordinary muscle ache.
