This piece lays out what the better studies found, how much confidence sits behind the numbers, why acupuncture is unusually difficult to test fairly, and why respected expert panels reading much the same evidence have reached opposite conclusions. We're not here to sell you on acupuncture or to talk you out of it β€” only to show you the evidence as it stands so you can have a sharper conversation with a regulated health professional.

What knee osteoarthritis is β€” and how acupuncture is used for it

Osteoarthritis is a degenerative joint disease in which the tissues of the joint break down over time [1]. It isn't only "worn cartilage": the changes can involve the cartilage capping the ends of the bones, plus the tendons, ligaments, the synovial lining, the bone itself, and β€” in the knee β€” the meniscus [1]. People commonly notice pain, stiffness that eases within about half an hour of moving, swelling after activity, and a sense of the joint being less steady [1]. It is also strikingly common: the World Health Organization estimates about 528 million people were living with osteoarthritis worldwide in 2019, with the knee the single most affected joint [2]. There is no treatment that reverses the underlying joint changes, so care centres on managing symptoms and keeping function β€” which is part of why non-drug options like acupuncture draw steady interest [1][2].

Acupuncture, as its practitioners describe it, involves inserting very thin needles into specific points on the body, sometimes with manual or mild electrical stimulation, across a course of sessions. In traditional East Asian medicine the points are framed in terms of restoring the flow of qi; many contemporary practitioners describe the same needling in terms of effects on nerves, local tissue, and pain-processing pathways. For knee osteoarthritis it is generally offered as an add-on aimed at easing pain, not as a claim to repair the joint. Those are the modality's own accounts of what it is and how it's meant to work; whether the needling delivers benefit beyond the ritual and attention around it is exactly the question the trials try to answer.

What the evidence shows

The standing high-quality synthesis is a Cochrane systematic review by Manheimer and colleagues, which pooled 16 randomized trials of acupuncture for peripheral-joint osteoarthritis, most of them in the knee [3]. Cochrane reviews sit at the top tier of evidence for how carefully they weigh each study's quality. And as with most fair tests, the answer swings hard on one thing: what acupuncture is being compared against.

Compared with fake ("sham") acupuncture. This is the toughest and most revealing test, because it tries to isolate the needling itself from the ritual, the attention, and the expectation that surround it. Here the measured benefit was small. For pain shortly after treatment (around 8 weeks), the review found a standardized mean difference of βˆ’0.28 (95% CI βˆ’0.45 to βˆ’0.11) β€” roughly 0.9 points on a 20-point pain scale, pooled across 9 trials and 1,835 people [3]. For physical function the figure was similar: an SMD of βˆ’0.28 (95% CI βˆ’0.46 to βˆ’0.09), about 2.7 points on a 68-point scale, which fell short of the review's own ~3.6-point threshold for a clinically relevant difference [3]. The authors' reading is the part to sit with: these sham-controlled benefits were "small, do not meet our pre-defined thresholds for clinical relevance, and are probably due at least partially to placebo effects from incomplete blinding" [3]. By six months, the sham-controlled difference in pain had shrunk further, to an SMD of βˆ’0.10 (95% CI βˆ’0.21 to 0.01) β€” which the review described as borderline and clinically irrelevant [3]. In plain terms: measured against a convincing placebo, the extra benefit of real needling looks small and fades.

Compared with a waiting list (no treatment). This is where acupuncture looked best β€” and also where the result is hardest to interpret. Pooling 4 trials and 884 people, the review found a large short-term improvement in pain (SMD βˆ’0.96, 95% CI βˆ’1.19 to βˆ’0.72, about 14.5 points on a 100-point scale) and a comparable gain in function (SMD βˆ’0.89, 95% CI βˆ’1.18 to βˆ’0.60) [3]. That is a clinically meaningful difference on paper. The catch is that a waiting-list comparison can't separate the needles from everything else that comes with receiving care β€” the sessions, the attention, and the expectation of getting better. The gap between this large waiting-list effect and the small sham effect is, in a sense, the whole story of this field.

Individual landmark trials bracket that tension rather than resolving it. In a 570-person U.S. trial, Berman and colleagues (2004) found that true acupuncture, added to usual care, produced modestly greater improvement in WOMAC pain and function scores at 26 weeks than either sham acupuncture or a health-education program [4]. A decade later, an Australian trial by Hinman and colleagues (2014) reached a different result: among 282 people with chronic knee pain, neither needle nor laser acupuncture outperformed sham treatment for pain or function [5]. Two carefully run trials, two different answers β€” which is precisely why a pooled review, and a healthy dose of caution, matter here.

Safety. The reported harms of acupuncture in these trials were generally minor β€” chiefly bruising, minor bleeding, or soreness at the needle site [3]. Serious harms were not a prominent feature of the osteoarthritis trials, though how well rare events can be detected in studies of this size is itself a limitation.

The sham-control problem β€” why this is so hard to study

Almost every disagreement in this field traces back to one stubborn design problem: building a fair placebo for a needle. In a drug trial you hand out an identical dummy pill and nobody can tell the difference. With acupuncture, the "placebo" usually means shallow needling, needling away from traditional points, or non-penetrating retractable needles β€” and the trouble is that these may not be truly inert. Touching or lightly pricking the skin can itself trigger real physiological responses, so the sham arm may deliver part of an active treatment rather than nothing at all.

That has a specific consequence worth understanding. If the sham isn't inert, then a trial comparing real acupuncture to sham may understate acupuncture's true effect, because both groups are getting something. The Cochrane authors pointed the other way as well: they judged that the small edge real acupuncture showed over sham was probably due at least partly to placebo effects from incomplete blinding β€” that is, patients and practitioners not being fully in the dark about who got what [3]. Both cautions can be true at once, and together they explain why the sham-controlled numbers are so slippery. Blinding is a genuine problem here: a patient might be kept unsure which needling they received, but the practitioner inserting the needles obviously knows. And expectation weighs heavily on pain β€” believing a treatment will help can, by itself, reduce pain, which is the very thing a placebo control exists to account for.

There's also a gap between the scientific question and the everyday one. A sham-controlled trial asks whether the needle itself adds anything beyond placebo β€” a real and important question about mechanism. But the choice a person actually faces is not "real needle versus sham needle"; it's whether or not to see an acupuncturist at all, attention and expectation included. Those are different questions, and the evidence answers the first far more cleanly than the second.

Where the guidelines diverge

Because the underlying effects are small and contested, expert panels weighing the same broad evidence have landed in genuinely different places β€” and this is one of the clearest examples in musculoskeletal care of guidelines disagreeing:

  • The American College of Rheumatology / Arthritis Foundation (2019) conditionally recommends acupuncture for knee osteoarthritis, describing it as worth considering for small improvements in pain and function [6]. A conditional recommendation is a soft one: it signals that the panel judged the balance of benefits, harms, low risk, and patient preference to tip slightly toward "reasonable to try," within shared decision-making β€” not that the treatment reliably works.
  • The Osteoarthritis Research Society International (OARSI, 2019) went the other way and recommended against acupuncture for knee osteoarthritis in its non-surgical management guideline [7].
  • The UK's National Institute for Health and Care Excellence (NICE, 2022) is the most explicit: its osteoarthritis guideline states plainly, "Do not offer acupuncture or dry needling to manage osteoarthritis" (recommendation 1.3.8) [8]. NICE's committee reasoned that, against sham, acupuncture showed no clinically important difference in pain or function [8].

Three respected bodies, three positions β€” conditional-for, against, and a firm do-not-offer. That spread isn't a sign that one panel blundered; it's what happens when the measured effect over a convincing placebo is small and uncertain, and reasonable experts weigh that small effect, its low risks, and its costs differently.

What this means for you

If you're living with knee osteoarthritis, here's a fair summary of where the evidence sits. Against a placebo, the measured benefit of acupuncture on pain and function is small, tends to fall below the threshold researchers call clinically meaningful, and appears to fade over months; against no treatment it looks larger, but that comparison can't separate the needles from attention and expectation; individual high-quality trials disagree; the reported risks are generally minor; and expert guidelines split three ways on whether to recommend it at all [3][4][5][6][7][8]. What that evidence does not support is a verdict in either direction. It would be overstating the research to call acupuncture proven effective for knee osteoarthritis, and it would be overstating it to call it proven useless β€” the honest position, and the one the platform holds, is that the benefit beyond placebo is small and genuinely contested.

None of that is a promise of relief, and none of it is a substitute for a proper assessment of your knee. The right next step is a conversation with a licensed, regulated health professional β€” your physician, a physiotherapist, or a regulated acupuncturist β€” who can examine your specific situation, rule out problems that need different care, and help you weigh acupuncture alongside options like exercise, weight management, and other therapies.
